Form Conversion Status

We are rapidly moving forms to web-based online services due to COVID-19. Please use the table below to see the status and location of SRRS forms.

Simple address changes should be completed online. If a student is changing their name in the system or making another advanced change, they should utilize the paper form. The paper form for these transactions should be accompanied by PII, and should not be emailed or uploaded.
Blackboard Observer Access Web-based Online form only. Reviewed and processed by SRRS staff and finalized with the Help Center.
Catalog Term Change Request Web-based Change of catalog terms can only be initiated by an associate dean, chair or advisor. The associate dean must approve. The form is completed and provided to the SRRS staff for action.
Change of Major Form Web-based (MRD)
PDF Format
Existing paper forms will continue to be accepted. New requests may be submitted using the online form. Department chairs or department staff must approve the form, which is routed to the SRRS staff for action. Departments are notified of the change when processed in order to send/share student files.
Co-Instructor Authorization Web-based Online form only. Approved by department and processed by SRRS staff.
College Withdrawal Form Web-based

Course Auditing Petition Web-based

Course Credit Overload Petition Web-based

Course Withdrawal Form Web-based

Degree Works Exceptions Web-based
PDF Format
Degree works exceptions can only be initiated by an advisor, chair or associate dean. The associate dean must approve. The form is completed and provided to the SRRS staff for action.
Directory Exclusion Request PDF Format Students may submit these online (MRD Upload) by scanning the document. No additional approval is needed. (This form is used infrequently.)
Faculty Change of Grade Form PDF Format (MRD Only) Currently, these may be submitted, scanned and uploaded via MRD. A web-based form is currently in development. This form include incomplete grading options.
Graduate Student Reactivation Form Web-based Existing paper forms will continue to be accepted. New requests may be submitted using the online form. Associate Deans and/or Graduate Coordinators will need to approve under certain conditions.
Incomplete Grade Agreement Form PDF Format Optional form used by faculty assigning incomplete grades.
Leave of Absence (LOA) Form Web-based

Existing paper forms will continue to be accepted. New requests may be submitted using the online form. Associate Deans must approve using the email approval process. Advisors are notified of student requests.
Medical Leave of Absence (MLOA) Form PDF Format These may only be initiated and approved by Campus Health Services or the Counseling Center and cannot be an online form presently. These may uploaded securely via myRedDragon by those offices.
Pass / No Credit Petition Temporary Web-based (MRD Only) Only online forms will be accepted for Spring 2020. The student will select the option, the advisor will be notified of the student choice.
Release for Certification of Professional Education PDF Format Students may submit these online (MRD Upload) by scanning the document. No additional approval is needed. (This form is used infrequently.)
Request to Change Graduate Status PDF Format Students may submit these online (MRD Upload) by scanning the document. No additional approval is needed. This form is used by some graduate programs electively. (This form is used infrequently.)
Schedule Change Form (Alternate Registration) Web-based (Visiting Students)
PDF Format
The schedule change form exists in both an online and paper form. Online submissions are accepted during drop/add periods and when a student is directed to use it. The paper form must be used after deadlines elapse or when an exceptional approval is made where a signature is required.
Student Information Release Form (FERPA Form) PDF Format Students may submit these online (MRD Upload) by scanning the document. No additional approval is needed. (This is a SRRS-specific form.)
TEACH Authorization Web-based Students may submit this form directly using the online process. (SRRS-specific form).
